Lubia serves as a Community Care Coordinator at Hanger Clinic in El Paso, Texas and Las Cruces, New Mexico. In her role, she provides comprehensive care and social services to local amputees through a structured, three-step process designed to support patients throughout their recovery journey.
Step 1: Initial Hospital Visit
Lubia meets patients shortly after surgery while they are still in the hospital, initiating their care journey. She meets with the multidisciplinary care team to ensure a safe and well-coordinated discharge for the patient.
Step 2: Community Resource Provision
Throughout the patient’s recovery, Lubia offers a wide array of community resources tailored to each person’s needs. These resources include transportation, nutrition assistance, Meals on Wheels, insurance, state benefits, education, advocacy, and more. Her personalized approach recognizes the unique challenges and recovery paths each patient faces.
Step 3: Peer Support Group
To maintain continuity of care, Lubia leads a monthly peer support group where members discuss challenges, explore new opportunities and services, and share experiences as they adapt to their new lifestyle. The group benefits from guest speakers from local agencies, providing ongoing access to resources and support as long as needed.
