Powered MPKAFOs

Powered microprocessor-controlled knee-ankle-foot orthoses help people with knee-related gait impairments walk more naturally and comfortably.

Smart Orthosis Supports Natural Motion

A powered microprocessor-controlled knee-ankle-foot orthosis (MPKAFO) is a smart orthosis that uses advanced sensors and joint motors to help people with knee-related gait impairments walk more naturally and comfortably. By supporting natural motion with real-time, phase-specific gait support, powered MPKAFOs are designed to help people walk more confidently, stand taller, and stay active longer.

First-of-its-Kind Bionic Power Agilik™

The Agilik is the first powered microprocessor-controlled knee-ankle-foot orthosis designed to assist mobility—not by walking for the patient, but by actively supporting natural gait. The lightweight actuator integrates into a custom-fabricated KAFO, creating a personalized system that moves with the patient.

How Powered MPKAFOs Work

Powered MPKAFOs are leg braces that use smart technology to help you walk more naturally and safely. They sense how you’re moving in real time and automatically adjust the support at your knee to give you better balance, stability, and comfort—whether you’re walking indoors, outdoors, or on uneven ground.

  • Functions by assisting or resisting knee extension and flexion throughout the gait cycle.
  • Monitors key aspects of leg movement during each step and uses this information to adjust the amount of assistance or resistance based on individual needs.
  • This targeted assistance helps improve knee extension, reduce fatigue, support upright posture, and promote strength.
  • It also collects gait metrics like range of motion, maximum flexion, and extension and angle speed to help clinicians make informed, data-driven care decisions.

Could You Benefit from a Powered MPKAFO?

In general, powered MPKAFOs are appropriate for those 5 years of age and older who experience:

  • Difficulty straightening the knee because of muscle weakness
  • Trouble standing up straight or walking with knees straight, especially over long distances
  • Instability or risk of falling while walking due to weak thigh muscles
  • Walking patterns that cause strain or pain in the hips, back, or trunk over time
  • Reduced walking endurance-not able to walk as far as before because of muscle weakness
  • Challenges practicing repetitive, consistent walking needed to improve posture and movement

Our approach centers on communicating with patients and their families, partnering with broader healthcare teams, and applying innovative technologies to ensure the best possible care today and into the future.

What to Expect on the Powered MPKAFO Treatment Journey

At the first appointment (which typically takes 60-90 minutes), your orthotist will evaluate, measure, and use tools and questionnaires to assess if a powered MPKAFO may be an appropriate solution for you.

Once candidacy is determined, the office staff will work to verify insurance coverage, with the goal of providing you with an understanding of their out-of-pocket costs (due to deductible and coinsurance).

You will start with a powered MPKAFO trial device. This adjustable system is designed to fit a range of heights and sizes, with various thigh cuffs and footplates for customization. During the trial, you can experience how the powered MPKAFO affects your walking, including comfort, stability, and the level of assistance provided. Your Hanger Clinic orthotist will monitor your gait and adjust the device’s settings to determine if the powered MPKAFO is the best solution for you

Upon completing fabrication of your customized powered MPKAFO, you will return to the clinic for fit and delivery. This appointment typically lasts 90 minutes. At that time, your orthotist will evaluate and adjust the brace to ensure a comfortable fit and the proper function. They will review the break-in schedule and cleaning instructions, show you how to take the brace on and off, advise how to care for your skin, and will schedule necessary follow-up appointments.

You will return 1-3 weeks following delivery for a follow-up appointment with your orthotist. At your appointment, the focus will be on ensuring the device is functioning properly, fits comfortably, and is helping you meet your mobility goals. Your orthotist will check that the brace fits well and doesn’t cause discomfort or skin irritation, analyze gait, and make any necessary adjustments to optimize settings. You’ll discuss how the device feels during daily activities, including any challenges and what is working well. Additionally, your orthotist will also take the time to answer questions and celebrate your accomplishments. Subsequent follow-up appointments will be made to continue ensuring proper care, function and servicing to best support your mobility goals.
